I dedicate this book to the memory of Mark Ault. Mark tragically died, too early, in a car accident some years ago. Every time I accomplish something in my chosen profession, I think of how much I’d like to share it with Mark, whose giant intellect and warm friendship were the constancies of my early university life. He was an academic advisor, skilled theoretician, critic, champion, and friend.
