This book, at least an early version of it, began in 2003 at the University of South Dakota where I worked with Susan Wolfe. She’s been a friend and mentor for many years and her initial ideas, as well as revisions of my early attempts to formulate a thesis on the subject, were instrumental. You can’t do anything without good teachers.
