There are so many people to whom I am indebted for assisting in various ways in the preparation of this study. In 2009 a number of us in the School of Education at Murdoch, inspired by Dr Rob Brown, set up a writing group that meets every six to eight weeks to provide feedback, ideas, and a quiet space in which to work on writing for publication. I am indebted to Sonja, Susan, Libby,
